work under the direction and supervision of the registered nurse to
conduct patient visits and provide routine care, while
communicating with the care team, the patient and their family.

A heart to serve patients and
families and a passion for providing the best possible
careEducation: Graduate of an accredited school of nursing with a
current state license as a licensed practical nurse Experience: 2+
years of nursing experience in an acute care setting, or equivalent
experience (hospice experience preferred) Required: Reliable
transportation. Ability to sit, stand, bend, move intermittently
and lift at least 25 lbs and bear the weight of an average adult
